The Southeast Bottle Club is sponsoring a contest to find what it's readers consider to be the top 10 North Carolina bottles in each category. This form is being used to collect nominations in each category. Please fill in up to 6 entries per category. Deadline for submission is December 31, 2003.

For each entry, include the full embossing on the bottle. If you need more, feel free to attach an additional sheet. And if you wish to attach pictures to your Email, that will be even more helpful and appreciated.
